Triskaidekaphobia (SOI-141) is a black rare enchantment from the Shadows over Innistrad set. At the beginning of your upkeep you choose one of two modes that causes any player at exactly 13 life to lose the game and then adjusts every player's life total by either gaining or losing 1. Official rulings clarify that the life adjustment still occurs even if no player would lose and that if all players have 13 life when the ability resolves the game ends in a draw.
