Its front face is a 2/2 flying Angel Soldier that can transform by paying 4 and a Phyrexian black cost (payable with black mana or 2 life). When transformed it becomes a 3/3 Phyrexian Angel whose attack trigger allows sacrificing another creature or artifact to give it +2/+1 until end of turn.
