Its front face creates a 1/1 white Human token and costs you 1 life whenever it or another nontoken Human you control dies, can pay {2} to give a Human lifelink until end of turn, and may transform at your end step if your life total is exactly 13 and you pay {4}{B}{B}. When transformed it becomes a 6/6 Demon with flying, trample, lifelink and an activated ability to sacrifice another creature to draw a card.
